About Us

Lweza Community Health Program (LCHP) is a not-for-profit Community Based Organisation (CBO), registered with Makindye-Ssabagabo Municipality. We are located in Lweza Cell B, Mutungo Ward, Ndejje Division, Makindye-Ssabagabo Municipality, Wakiso Distrcit, UGANDA.
LCHP was formed in 2020, by serving and retired health professionals, with the core objective of promoting primary health care in the community.
LCHP works with local authorities and the community to addresses issues that affect health e.g. nutrition, immunization, substance abuse, garbage disposal, domestic violence, child abuse, etc
At Lweza Community Health Program (LCHP), we are guided by the following core values:

Following are some of the volunteers behind Lweza Community Health Program.
Rose Kiwanuka
BoD Chair & Founder
Rose is a retired Palliative care Nurse Specialist, founding member of Hospice Africa Uganda and founding Country Director Palliative Care Association of Uganda (PCAU). Over the years, she has supported the development of palliative care in Uganda.
Hajji Ssali Muhammad
BoD Member & LC1 Chairman
Hajji Sssali is the elected Chairman of Lweza Cell B Local Council 1. He is a hands-on chairman, very enthusiastic about all issues of the community - from health to education to sanitation to security to infrastructure among others. He represents the Local Council on the LCHP BoD.
Karamagi Frederick
BoD Vice Chair
Frederick is a retired teacher of Mathematics & Physics and an IT Trainer. He carries years of experience in telecommunications and IT. He is passionate about using technology to improve healthcare in the community.
Dianah Basirika
BoD Secretary & Founder
Dianah is a Public Health Specialist and a nurse with vast experience in Palliative Care in Uganda and internationally. She is very passionate about community health and promotes health equity through Primary Health Care.
Ritah Nannyomo
BoD Treasurer
Rita is a Finance Officer at the Palliative Care Association of Uganda (PCAU). She carries years of experience in Financial management and accounting, and is very passionate about people's health and well - being.
Fatia Kiyange
BoD Member
Fatia is the Executive Director of Center for Health, Human Rights & Development (CEHURD) with 20+ years of experience in health & social welfare. She is a passionate advocate for control, management and care. She previously worked with Africa Palliative Care Association (APCA) and Hospice Africa Uganda in the field of palliative care at national, regional and global levels.
Owino Alex
BoD Member (Youth)
Alex is a tax consultant, software integrator and accountant. He is active in healthcare activities in the community. Alex represents the youth on LCHP BoD.
